Who we are
Langford Chiropractic operates a chiropractic clinic at U 1/2 View Avenue, Langford WA 6147. This policy explains how we collect, use, disclose and protect your personal and health information, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
What we collect
We collect the information needed to provide safe and effective care: your name, date of birth, contact details, Medicare and private health fund details, emergency contact, and your health history including symptoms, examination findings, treatment provided and correspondence with other practitioners. Health information is classified as sensitive information and is given a higher level of protection.
How we collect it
Usually directly from you — through your new patient form, your consultations and your phone or email contact with the clinic. We may also receive information from your GP, specialist, another treating practitioner, an insurer, or a parent or guardian where you are a child.
Why we collect it
To assess, diagnose and treat you; to keep accurate clinical records as required by law and our professional obligations; to process health fund, Medicare, WorkCover and insurance claims; to contact you about appointments; and to meet legal, accreditation and regulatory requirements.
Who we disclose it to
Only where you have consented, or where the law requires or permits it. That may include other practitioners involved in your care, your health fund or Medicare for claiming, an insurer for a workers compensation or motor vehicle claim, and our practice software and IT providers who are bound to keep the information confidential. We do not sell your information, and we do not disclose it overseas without your consent.
Booking and online forms
Appointments are booked through Cliniko, a third-party practice management platform. Information you enter into the booking system or an online form on this website is transmitted to and stored by that provider under their own privacy terms. This website may also use analytics cookies to understand how visitors use the site; you can disable cookies in your browser at any time.
How we store and protect it
Records are held electronically in secure, access-controlled practice management software, and any paper records are stored securely on site. Access is limited to staff who need it to do their job. Health records are retained for at least seven years from the date of last service, or for a child, until they turn 25 — whichever is longer — after which they are securely destroyed.
Accessing and correcting your information
You can request access to your health record, or ask us to correct information that is inaccurate, by contacting the clinic in writing. We will respond within 30 days. An administrative fee may apply for producing copies. In limited circumstances we may refuse access, in which case we will explain why in writing.
Complaints
If you believe we have mishandled your information, please contact the clinic first so we can try to resolve it. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner at oaic.gov.au, or the Health and Disability Services Complaints Office (HaDSCO) in Western Australia.
Changes to this policy
We may update this policy from time to time. The current version is always available on this page, with the last-updated date shown at the top.
